Information Needed to Make Arrangements

The following information will assist you when making funeral arrangements:

  1. Arrangement appointment time.

    To Assure the availability of one of our directors, please call and schedule a convenient arrangement appointment time. ( Not shown: Appointment Form -- See downloadable PDF file. )
  2. Data needed to prepare the Death Certificate, which is required by law.

    Please complete the included checklist and bring to the arrangement meeting.

  3. Other helpful information to be brought in when making arrangements.

  4. Clothing.

    A full complement of clothing is utilized, including undergarments and outer clothing of your choice. Shoes are optional. For women, an outer garment with long sleeves and high neckline is desirable.

  5. For your assistance.

    Please feel free to call us anytime, as there are personnel on duty 24-hours a day and there is always someone available to take a telephone message.

  6. Personal effects.

    If there are personal effects at a hospital or the coroner's office, you will need to make arrangements to pick them up, as Palm is not authorized to pick up personal effects.

Flowers

Flowers are available for all occasions and can be delivered locally or out of town through our wire services. Should you desire us to handle your flower needs for the funeral service, cemetery or mausoleum, just call (702) 464-8400. All major credit cards are accepted by phone.

Flower Placement Service

We would like to call your attention to our flower placement service for local cemeteries or mausoleums in Las Vegas and Henderson.

Flowers are the universal expression of loving remembrance on those special days that mean so much. We offer a helpful floral service on these occasions because it is not always convenient for you to place flowers personally. Same day placement service is available.

Our flowers are guaranteed fresh and are reasonably priced. Fresh cut flowers may be placed any time throughout the year. For the flower policy in our cemeteries, see the cemetery rules and regulations section in this brochure.

Crematory Rules and Regulations

Printed copies of our cremation policies and procedures are available at any of our mortuary locations. Following are answers to some commonly asked questions about cremation procedures.

The Nevada Revised Statute (451.650) defines the order of priority of persons who may order the cremation of another person as follows:

  1. A person designated as the person with authority to order the cremation of the human remains of the decedent in a legally valid document or in an affidavit executed in accordance with subsection 5 (which provides an example of such an affidavit);
  2. The spouse of the decedent;
  3. An adult son or daughter of the decedent;
  4. Either parent of the decedent;
  5. An adult brother or sister of the decedent;
  6. A grandparent of the decedent;
  7. A guardian of the decedent at the time of death;
  8. A person who held the primary domicile of the decedent in joint tenancy with the decedent at the time of death.

Palm Mortuary’s policy is to require that all persons having the same priority must complete and sign the Cremation Authorization and related documents.

Cremation can take place only after the following conditions have been met:

  1. Identification of the decedent has been verified;
  2. Civil and medical authorities have issued all required permits;
  3. All necessary authorizations have been obtained, and no objections have been raised.

Cremation is not a final disposition of the human remains. It is not a funeral service. Any type of remembrance, memorial service or traditional funeral service may be arranged. Cremated remains are the remaining bone fragments resulting from cremation. There are several ways to memorialize and provide a permanent resting place for cremated remains after the cremation.

For example:

  • You may have cremated remains placed in a garden or building niche in a cemetery with a plaque or marker to memorialize the individual.
  • You may have cremated remains scattered in a designated area in a cemetery. In addition, you could have the individual’s name listed on a plaque with others scattered in the same area.
  • You may take cremated remains with you or you may have them sent to you by registered mail.

There are various types and styles of urns (permanent containers for cremated remains) available through Palm Mortuary for memorialization and/or retention.

Cemetery Rules and Regulations

General Information

Palm Mortuary, Inc., was organized as a private cemetery corporation in 1957 and presently operates five Memorial Parks.

In order to properly care for and maintain the beauty in our Memorial Parks, it is necessary that certain Rules and Regulations be adopted and enforced. In addition, Rules and Regulations have been adopted to secure uniformity within the Memorial Parks and to protect the interests of all of the Owners of interment space in the Memorial Parks.

The Cemetery Rules and Regulations governing Palm’s Memorial Parks have been designed, and will continue to be designed, to be of benefit to the Owners of interment spaces collectively. Palm may, and hereby expressly reserves the right to, at any time, adopt new Rules and Regulations or amend, alter, or repeal any existing Rule or Regulation, without notice to the Owners.

The following partial information is furnished for your clarification; complete Rules and Regulations governing our Memorial Parks are on file in our offices.

Memorial Park Hours

General Rules for Park Visitors

All visitors to the Memorial Parks must display proper respect for the deceased. When on any of the properties, please act accordingly with care and consideration for all. Soliciting, peddling, or distributing literature while on any of Palm’s Memorial parks is strictly prohibited.

Flowers

Palm understands and respects the need for families to show respect and love through the placement of floral offerings and special occasion mementos. In the interest of uniformity, consistency, and in order to maintain the beauty of all Memorial Parks, Palm requires that all floral arrangements, memorabilia, or other personal items placed at the gravesite be secured in an approved permanent lawn or mausoleum vase receptacle. Styrofoam, supplied by Palm, may be used to secure artificial flowers, flags, ornaments, and other personal items in the vase. For assistance, please contact any of our cemetery employees.

Funeral Flowers

Funeral flowers, along with all other funeral items, such as stands, frames, baskets, and casket sprays, will be removed from the interment area within two (2) days of the funeral. Family members should remove any funeral items they wish to take with them at the conclusion of the funeral service.

Special Occasion Flowers

During the Christmas season (December 1 through January 8), special occasion flowers and mementos may be placed on gravesites. In addition, during the Easter, Memorial Day, and the 4th of July holiday periods, special flowers and mementos may be placed up to seven (7) days prior to the actual holiday and left for up to seven (7) days after. However, we ask that during this time, only one special arrangement per gravesite be placed that does not fit in the permanent vase receptacle.

Removal of Flowers

Palm will remove without notice any items not placed in an authorized vase receptacle except during the special holiday periods as listed above. In addition, flowers contained in vase receptacles that are deemed, at our discretion, worn or unsightly will be removed. Under no circumstances are open flames or anything combustible allowed in the Memorial Parks. They will be removed immediately.

Decorations/Ornaments in Mausoleums and Columbarium

Photographs, souvenirs, cards, wreaths, and other items are prohibited on mausoleum and columbarium facings. Such items will be removed by Palm, and, if sufficient damage has been done to the face of the marble or glass due to the items placed, the face will be replaced and the cost charged to the crypt owner.

Continuing Care

Our commitment to you and your family extends far beyond the funeral service. We know from experience that you will have many questions and concerns to deal with after your services have taken place.

We want you to know that your funeral director will be available to assist you following the funeral service. In addition, a member of Palm’s Family Services staff will be made available to you and your family. They will visit with you following the funeral services to help you with any needs you may have.

Your Family Service representative will also be available to assist you with any questions about Palm’s policies and procedures, flower placement regulations, or property arrangements. Please feel free to call upon your representative to answer your questions and attend to your needs at any time.

We are grateful for the trust and confidence you have placed in all of us at Palm. There will never be anything more important to us than caring for your family at this difficult time and providing a meaningful tribute service and dignified place of rest for your loved one.

Your Family Service representative is also trained and licensed to aid your family in the area of advance planning. Families who have experienced the loss of a loved one often find it valuable to document their own thoughts and wishes. Please feel free to discuss any questions you have about Palm’s services with your representative.

Acknowledgments

  • Etiquette Cards or Folders

    No longer are personal letters expected by relatives, friends, and acquaintances in return for expressions of sympathy. In most cases a signed Acknowledgment Card or Folder is sufficient to say “Thank You” for flowers, Mass cards, Sympathy cards and personal services.

    Acknowledgments should usually be sent within ten days to two weeks following the funeral.

    If a more personal touch is desired, a few words can be written on the Acknowledgment Folder with reference to flowers, letters of condolence or to the services that were rendered. These words make the recipient feel that one’s thanks are as personal as if they had been expressed verbally.

    We hope the following suggestions will prove helpful in handling the various expressions of sympathy the family receives during bereavement.

    The notes printed below in italics are only suggestions. Feel free to change any wording to fit your situation.

  • Clergy

    You may want to send a personal note thanking them for their spiritual help.

    Dear Reverend Smith:

    My husband and I thank you for the consolation you gave us during this trying time. The beauty of the services you conducted will help us tremendously.

    Dear Father Quinn:

    Thank you for the consolation you gave us and for your continuing prayers.

  • Pallbearers

    Include a message of thanks for their time and services on an Acknowledgment Folder.

    Thank you for your kindness in acting as a Bearer for Father. We asked you because you were one of his closest friends.
  • Letters

    Replies may be short and written on an Acknowledgment Folder.

    Thank you for your kind words of sympathy...it is so nice to have friends like you.
  • Flowers

    A personal message may be written on the Acknowledgment Folder.

    The roses you sent for Dad were beautiful.
  • Flowers Received from an Organization

    Send an Acknowledgment to the leader of the group and include in the message reference to the other members.

    The flowers sent by the members of the East Side Rotary Club were beautiful and greatly appreciated.
  • Flowers Received from an Unorganized Group

    When a group of neighbors, employees, etc., sends flowers, an Acknowledgment to each name on the card is sufficient.

  • Friends who Volunteer Cars and Services

    A personal note on an Acknowledgment is appropriate.

    • Car:
      Thank you, Bob, for the use of your car. It was needed and greatly appreciated.
    • Service:
      You were so kind to stay with Mother after the funeral and helping her through that first evening alone.
    • Food:
      The apple pie you sent to the house was delicious. Thanks again for your thoughtfulness.

Social Security & Veteran's Benefits

Social Security Benefits

The following information is provided by the Social Security Administration to help you obtain any Social Security benefits to which you may be entitled. Please follow the steps below to determine eligibility for filing a claim. Social Security benefits are payable directly to the family and do not satisfy any credit arrangements with Palm.

The Social Security Act provides for the payment of survivor’s benefits to certain relatives of a deceased worker. If you are one of the following, you could possibly be eligible for a benefit:

  1. Surviving spouse living with the Social Security number holder at the time of death.
  2. Widow(er) age 60 or above.
  3. Widow(er) age 50-59 and disabled.
  4. Mother (Father) with a child 16 or under of the deceased worker in his/her care.
  5. Unmarried children of the deceased worker age 18 and under or 18-19 and in high school.
  6. A disabled child whose disability began prior to age 22.

In addition, a surviving divorced spouse may be eligible for benefits in certain situations. Please contact Social Security for further information concerning surviving divorced spouses’ benefits.

You should CALL the Las Vegas Social Security Office to see if you qualify for benefits. PLEASE CALL FIRST. The entire process of filing and establishing eligibility can be done by telephone and through the mail.

Veteran's Benefits

The Veterans Administration DOES NOT provide any payment for funeral services. The Veterans Administration does provide PARTIAL payment to certain Veterans, with a discharge other than dishonorable, and to service members who die while on active duty, for certain cemetery costs. Generally, only Veterans who served in WWI, WWII, the Korean conflict and the Vietnam conflict are eligible for these benefits. Two types of benefits may be available:

  1. Partial reimbursement for burial expenses, if the veteran qualifies. These reimbursements are made directly to the family when the funeral account is paid in full. When there is a balance due on the funeral account, the Veterans Administration will send payment to the mortuary. If there is a credit balance after benefits are received, the family will receive a refund.

  2. A particular type of memorial (marker) is provided by the government. Palm has a cemetery section, The Garden of Honor, for Veterans and their families. This garden uses the government provided markers. When the marker is ordered from the Veterans Administration, it could take up to one year to receive the marker.

Your arrangement director at Palm will be happy to fill out he necessary paperwork for you.

Full military burial honors are provided strictly at the discretion of the military. They are generally provided only to those on active duty, retired military personnel or disabled.

All discharged Veterans, other than dishonorable, are entitled to a United States Flag for their services free of charge. Following the services, if you choose, the flag may be donated to Palm Memorial Park and it will be flown on the cemetery grounds on those days designated for flying flags: Memorial Day, Flag Day, Fourth of July, and Veterans Day.

All Veterans and dependents are responsible for notifying the Veterans Administration as soon as possible of any change that could affect their VA monetary benefits.

Payment Policy

Palm Mortuary has established a uniform payment policy to serve all families fairly and to prevent misunderstandings. This payment policy enables us to contain our costs for all the families we serve.

Palm’s policy requires that all financial arrangements must be completed prior to the setting of services or viewing times. Payment for our merchandise, services, and property is due one full day before the service time or the use of our merchandise or property.

Palm does not accept future payment from estate funds.

The following are acceptable payment arrangement:

  • Cash. Checks are subject to electronic funds verification. Third party checks are not acceptable.
  • VISA, MasterCard, Discover, or American Express.
  • Assignable insurance, for the face value of the policy only, once the assignability and beneficiary have been verified by the insurance company. Policies under two years old are generally contestable, and may not be acceptable. When acceptable insurance has been assigned, a monthly statement will be sent to the purchaser until payment is received from the insurance company. When payment is received, a statement will be sent showing the amount, date, and indicate whether the funeral account is paid in full or a new balance is due. There is an administrative fee for processing assigned insurance policies, and the normal processing time for a policy is 4 to 6 weeks.

About Death Certificates

Certified death certificates are legal documents provided by the Clark County Health District for a fee. It is extremely important that you provide us with correct information for our preparation of the original death certificate.

Photocopies without the certified seal from the Health Department are not accepted as legal documents. During the arrangement process, we can order the number of copies you may need.

After the original death certificate has been filed, additional certified copies of the death certificate are available, for a fee, from the Health District. Palm will be happy to secure them for you, or you may obtain them while you wait at the Clark County Health District, 625 Shadow Lane, Las Vegas, Nevada 89106.

It Generally Takes Ten Working Days to Obtain Certified Death Certificates.

When we order them for you, we will notify you when they are ready so you may pick them up, or we will send them via Federal Express at your request. Listed below are some common areas requiring a certified copy of the death certificate:

  • Social Security AdministrationLife insurance policies and other insurance policies
  • Union benefits, pension or retirement funds
  • For accounts at each financial institution
  • For international shipment of remains
  • Home mortgages
  • Stocks, bonds, etc.
  • Titles and deeds to property
  • One for your own records
  • Veteran’s Administration (currently provided free of charge by the Health Department)
  • Vehicle registration & title changes
  • CPA and attorney
